Attending Nurse Practitioner

Hours:

Full Time Days

Qualifications:

  • Master’s Degree in Nursing/Current extended class registration RN (EC) required with CNO
  • Demonstrated patient/family teaching and staff education
  • Exemplary clinical skills, knowledge and expertise
  • Recent and relevant experience practicing as a Nurse Practitioner, geriatric experience preferred
  • Experience in chronic disease management, dementia care and wound management an asset
  • Proficiency in computer skills (e.g. Excel, Word, PPT) and data management required
  • Excellent organization, communication and interpersonal skills
  • Required to work in and travel between two LTC homes in Shelburne, ON
  • Excellent critical thinking, problem solving and organizational abilities
  • Demonstrated ability to deal effectively with sensitive issues with families and staff
  • Demonstrated knowledge and/or experience working within a professional practice and patient focused care environment
  • Evidence of initiative in ongoing knowledge development and reflective practice
  • Fluent in English (comprehension, reading, writing, and speaking)

JOB SUMMARY:     

The NP will demonstrate a high level of autonomy and expert skill in formulating clinical decisions and appropriately managing a population of patients within two Central West LTC facilities (220 beds in total). The principal responsibilities will include: utilizing and demonstrating a comprehensive, advanced level of clinical knowledge and competencies, collecting and interpreting medical information about the health of the resident. The NP will work to their full scope of practice in very busy, dynamic LTC homes providing acute episodic care to residents to avoid unnecessary ED transfers. You will play a leadership role and perform quality improvement; participate in policy and procedure development and education activities/programs; act as a resource and serve as a consultant to individuals and groups within the professional community and other external stakeholders associated with the individual LTC homes. The Nurse Practitioner will facilitate educational support to residents, families and staff in the LTC facility. The Nurse Practitioner will also attend facility meetings and resident care conferences as needed.

The NP role involves close collaboration with stakeholders to facilitate change and promote staff and resident/family satisfaction within a philosophy of patient-centered care.

Part-Time Personal Support Worker (PSW)

Retirement Living and Long Term Care

ROLE:

Provides bedside nursing care to residents under the supervision of the Registered Staff/Resident Services Coordinator /Director of Care, as a member of the Multidisciplinary Team.

QUALIFICATIONS:

  • Completion of a certified Personal Support Worker Program. Certificate to be on employee file, including any updated certificates.
  • Experience in Long Term Care is preferable.
  • Possesses aptitude and compassion necessary for working with the aged and infirm.

  • Flexible hours to accommodate day, evening and weekend programs.
  • Maintain an awareness of facility safety and compliance:

Part Time – Retirement Home Attendant
Retirement Living and Long Term Care

ROLE:

  • To assist Residents to maintain (as much as possible) their level of independence and care requirements.
  • To give assistance when needed to each Resident individually for personal, physical, emotional, and spiritual needs.
  • To help maintain the cleanliness of the facility in all areas.

BACKGROUND/QUALIFICATIONS:

Ability to follow verbal and written instructions.

Ability to document information received in written format.

Able to work with seniors and people with disabilities.

Previous work experience in a health care facility is preferable

Certified in first aide.

WORKING CONDITIONS

Required to remain on your feet for several hours at a time. Able to climb stairs. Able to take direction from others.
